The mission of the Total Health Care Center for Emotional and Social Health (CESH) is to improve the health and quality of life for all, regardless of ability to pay. Goals of Total Health Care CESH

Through the provision of the relationship with a licensed mental health professional and at the request of the patient, is

1. To help the patient to learn, set and achieve life-enhancing goals that allow the patient to experience excellence in their lives.
2. To help the patient to live successfully and at a satisfactory level within their families and communities.
3. To support and enhance the healthy bond within the family and community system so that prevention and wellness define the needed services.
4. To promote wellness within the relationship between the patient and their family and their community, ideally achieving healthy growth for each person. Provide support for the healthy choices the patient makes in day-to-day existence.
5. To help the patient become aware, involved, and informed managers of their own lives, so they become effective advocates for themselves and their community, including their interpersonal and social lives. /li>

Description of Services

Total Health Care’s CESH program provides comprehensive mental health services to adults, children, and families in Baltimore. The services are geared to individuals whose needs can be met in an outpatient setting. The patients’ needs may be chronic in nature but stabilized enough to be managed in a community setting. More complex mental health needs include those requiring hospitalization, are not treated in the Mental Health Clinic.

Total Health Care’s CESH program provides both psychotherapeutic and psychiatric services. Psychotherapeutic services include individual, couples, and family therapy; crisis management; psychosocial assessment; psychological evaluation and educational testing. Psychiatric services include evaluation and medication management.

The actual work occurs within the quality of the relationship between the psychotherapist and the patient. The psychotherapists receive ongoing support, training, and supervision from professional staff. The psychotherapists also provide:

1. Highly individualized treatment planning
2. Liaison and coordination of multiple services via a mental health case management approach
3. Training and support to family members via psycho-educational services
4. Multi-disciplinary evaluation and coordination of each patient’s progress and quality of life via participation in the comprehensive treatment teams who monitor the delivery of competent clinical services
5. Services to an ever-expanding region

Population Served

Total Health Care’s CESH program serves males and females who range in age from 5 to 99 years. Patients may present problems with fearfulness, unresolved grief, aggression, drug abuse, promiscuity, delinquency, social withdrawal, or any number of behaviors that lead to a cycle of bad choices that make their lives miserable.

Patients present with symptoms and behaviors consistent with diagnosis from the DSM-IV, such as (but not limited to):

1. Affective disorders
2. Suicide ideation, suicide attempts, or ideation in their past
3. Personality disorders
4. Attention deficit disorders
5. Anxiety disorders
6. History of and abuse of alcohol, drugs and over the counter substances
7. Learning disabilities
8. Sexual disorders
9. Self-mutilation and abuse
10. Abuse victims and perpetrators (including sexual abuse)
11. Adjudicated delinquency and criminal behavior
12. Fire setting history
13. Impulse control disorder

Referral Process

The patient’s physician or another social service agency representative may make the referral for services. Also, patients may self-refer by calling the clinic directly or by requesting a referral from their physician.
